In this blog we are going to talk about therapies for male pattern baldness, specifically Finasteride treatment. Many people come to us and tell us that they have tried many things for hair loss, but non of them are working. We we ask them what happened when they stopped the therapy, they tell us that it got worse.

When clients come to us wanting hair transplant surgery, one of the first questions we ask is; "Are you taking medical stimulation and blocking therapy?". Finasteride treatment is known as a blocking therapy as it blocks the production of DHT which causes hair loss in susceptible individuals.

This brings us to our first major point and that is; "success in usage of hair loss therapy should be defined as not losing more hair". If your hair is not getting any thinner, then that can be considered as the first level of success.

A greater level of success may be defined as an improvement in hair thickness. This is possible in some cases but not all cases and will be depend largely on how soon the client presents to us with a hair loss concern. Once balding has set in the hair follicles are no longer viable and no therapy is able to resurrect them. Realistically we are better at stabilising hair loss rather than increasing and improving the density of hair. It may be possible for some people but not for everyone.

Without having any therapy it is guaranteed that in twelve months time your hair will be thinner than what it is today. In 1988 Finasteride came onto the market and in 1989 Minoxidil came onto the market. These two drugs have revolutionised the therapy of male pattern baldness.

In most men, reducing the level of DHT by 2/3 is enough to stabilise their hair loss. In other people because of their different genetics, they will continue shedding hair.

A good analogy is a bucket of water with a hole in the bottom of the bucket. The role of Finasteride treatment is not to add more water to the bucket, but to help plug the hole up, so that the rate of water loss is reduced. Finasteride may not block up the hole completely, but it will certainly help to reduce the flow of water out of the bucket.

Finasteride must be considered as a treatment for hair loss rather than a cure for hair loss. For you to benefit from Finasteride, you will need to continue taking Finasteride over a long period of time. Finasteride is essentially fighting your genes and fighting the five alpha reductase enzyme. You should only continue taking Finasteride for as long as you continue to care about hair loss. Many men once they reach fifty no longer care so much about their appearance or hair loss. In a small percentage of people it is possible to achieve stability without hair loss for 20 years by using Finasteride regularly.

Finasteride treatment is a specific therapy targeted only at male pattern baldness and has revolutionised the treatment of male pattern baldness. Finasteride has no side effect on the body's organs and is considered a very safe medicine. Another benefit of Finasteride is that it has no interaction with any other types of medications. Finasteride is safe and an effective treatment for male pattern baldness.

Just to dispel a few myths; some people believe that Finasteride treatment will increase their testosterone and help their libedo. It is true that Finasteride treatment may increase testosterone by 10% but this will have no significant effect on their libedo. There is a lot of misinformation on the internet about the side effects of Finasteride. However when Finasteride is taken at the recommended doses, it does not have any significant side effects. The side effects of Finasteride will be explored more deeply in a subsequent blog.

Minoxidil Treatment for people suffering Androgenic Alopecia in Adelaide

At Advanced Cosmetic Medicine, we always like to advise clients that are suffering from hair loss to preserve their existing hair for as long as possible. Both blocking and stimulating therapies cannot halt the progress of Androgenic Alopecia, however they can go a long way towards slowing down the rate of hair loss. Also, when clients come to us wanting a hair restoration procedure, it is a requirement they they take blocking and stimulating therapies to slow down the loss of hair. Donar hair supply is a finite resource, so existing hair should be maintained for as long as possible.

There are two types of medical treatment for androgenic alopecia.  These are known as “blockers” or “stimulators”. Minoxidil is classed as a form of stimulation therapy.  It does exactly what it says, it stimulates hair growth.

Stimulators, however, do not address the underlying cause of androgenic alopecia. All they do is stimulate growth of the hair.  Minoxidil has been on the market for many years and for a long time was the only treatment available for hair loss. Minoxidil treatment was originally designed for use as an oral tablet for treatment of high blood pressure.  

One of the side effects that was discovered from its use is that recipients became much hairier.  In fact 50% of people prescribed this treatment were found to become hairier.  The pharmacology companies then decided to make Minoxidil into a topical agent that can be applied directly to the area of hair loss.  The exact mechanism of action of Minoxidil still remains unclear, although it is thought to have something to do with opening up potassium channels. Minoxidil is known to be a safe therapy.  Topical Minoxidil treatment is an over the counter drug which can be purchased from any pharmacy without a script.  Most over the counter drugs are known to be very safe if used in the correct manner.

Minoxidil treatment does two things.  Firstly, it is known to lengthen the anogen phase of hair growth.  In other words more hairs are in the actively growing phase of the life cycle at any one time and less hairs are shed.  A small percentage of people will notice an increase in shedding rate when they first start Minoxidil. This is a temporary phase caused by the new hair pushing the old hair out. This is a good thing not a bad thing.  After starting on Minoxidil it will take from 4 to 6 months for you to start noticing an improvement in your hair thickness.  People often try Minoxidil and think that it has been a failure because they have noticed the original shedding reaction and they have not noticed any benefit. People need to be patient and wait 4 to 6 months to be able to see any benefit. 

The first goal of stimulation therapy is to achieve stability and halt the continual loss and miniaturization of hair.  The goal of medical therapy is stability.  A good analogy is a leaking bucket.  In order to stop the bucket leaking we need to plug up the hole.  40% of people that use topical Minoxidil achieve positive results.  The most improvement comes from an increase in the shaft diameter of the hair. There is also a small increase in the number of hairs present.  The major benefit of Minoxidil is to increase the thickness of the hair shaft.

Minoxidil treatment can be used for any type of hair loss eg loss of hair after giving birth, use of medications, after an acute stressful event or from immune conditions like alopecia areata.  40% of people using Minoxidil over time will notice an improvement.  Another 40% of people over time notice that they are not getting worse.  This is the second benefit of Minoxidil.  So 80% of people are going to use Minoxidil with some sort of success.  The only real way to know that Minoxidil is working is to stop taking it and see if you continue to lose hair and lose hair density.

Topical Minoxidil treatment only needs to be used once a day.  It can last in the skin for 21 hours.  1ml per day is usually sufficient.  The actual quantity of Minoxidil used is not so important; the most important thing is to cover the area that needs to be treated.  Minoxidil comes in two different strengths, 7% and 5%.  The dose response difference between 7% and 5% is not significant. You will not notice any difference in the response between having 7% or 5% Minoxidil.  There is also a mens and womens Minoxidil treatment.  Womens Minoxidil comes in two strengths 5% and 2%.  Women may experience and increase in beard growth using 5% Minoxidil.  If this occurs, reducing the dose down to 2% will reverse this effect.  Stopping the treatment will also stop the beard growth. 

Many people do not like topical Minoxidil treatment because it can make the hair oily and the scalp itchy. There are two types of topical Minoxidil on the market, there is a liquid and a foam. For the liquid Minoxidil the base preparation is Propylene glycol, which is what causes the itchiness and flaky scalp in about 30% of people that use it. The foam is better tolerated by most people and is less greasy because it does not contain PG. Minoxidil should only be applied to the scalp.  There is no benefit from applying it to the hair.  Foam is also much faster to absorb through the skin, taking only ten minutes. The liquid base may take up to sixty minutes to absorb through the skin.  The foam only needs to be applied for fifteen minutes before doing sports or washing your hair.

In summary, Minoxidil treatment has been around for a long time, it is known to be safe, it is a good stimulator of hair growth, it comes in a liquid and a foam, the 5% Minoxidil treatment is applicable to most people, most people don’t react to the foam, taking it once a day is sufficient, in some people there will be an initial shedding phase after first taking it.

Oral Minoxidil treatment can also be taken, but is a prescription medicine only.  People may not like topical Minoxidil for styling reasons. For these people a very low dose of oral Minoxidil may be prescribed , which will not affect blood pressure.

Minoxidil treatment may help to slow down hair loss in androgenic alopecia but will usually not halt the progress of hair loss completely.  Minoxidil should be used as a long term therapy to stabilise hair loss.  For best effect, blockers and stimulators can be combined.  Blockers will be discussed in a separate blog at a later stage.

It does happen that from time to time we get a client that asks the question, "Why Have My Anti Wrinkle Injections Stopped Working?"

Australia has one of the largest cosmetic services industries worldwide, having recently surpassed America in number of procedures per capita. All over the country, we are forking over our hard-earned cash to enhance the way we look. In fact, Australians spend an estimated $350 million on anti-wrinkle injections annually. After all, it’s fast and relatively painless. More importantly, it is a sure-fire way to erase the worries from our faces and achieve serene expressions – or is it?

For most who receive this procedure, the anti-wrinkle injection product freezes and relaxes the muscles, preventing them from contracting and forming new wrinkles, and softening lines and wrinkles that have already formed prior to treatment. In four to six months, our bodies break down the product completely, which necessitates a follow-up treatment.

But there is a subset of those for whom it may seem as if the treatment isn’t achieving the desired results or the effects of the treatment are wearing off faster than usual. This can occur for both first-time users and regular users of this cosmetic treatment. It can be frustrating not knowing the reason for this and wondering whether the product was used in the right dosage or whether the practitioner administered the product correctly. It turns out, however, that our bodies might be to blame.

Studies have shown that an estimated 0.3 to 6% of us are immune or develop immunity to the effects of anti-wrinkle injections. While this may seem like a negligible percentage, the sheer size of the industry means that this small ratio translates to literally thousands of people not getting the effects they desire.

In plainer terms, immunity to anti-wrinkle injections means our bodies are recognising the injection product as a foreign substance and producing antibodies to fight it, thus reducing the effectiveness of the treatment. This is because the active ingredient in anti-wrinkle treatments is a type of protein, a paralysing neurotoxin. When introduced into our muscles, our bodies can identify it and induce an immune response. This likelihood increases and can become an issue if these treatments are used frequently and repeatedly over time or are needed to treat medical conditions such as dystonia (a condition in which a person’s muscles twitch uncontrollably).

Fortunately, a way around this is to receive smaller doses of the anti-wrinkle product and leave the longest possible interval between treatments. This can reduce the possibility of our bodies becoming resistant to the treatment.

How to wash the donar area:

In the afternoon following your hair transplant you can take off the bandage and wash the back of your head. Here are the steps required for washing your donor area:

  1. Prior to starting see if you someone can help you, if not ensure you have a mirror to hand in the bathroom.
  2. Ideally you want a shower head which you can hold in your hand and adjust the pressure, if not running the tap and using a clean cup will also work.
  3. Run the water ensuring that it is lukewarm, if you are using a shower ensure the pressure is set to medium/low.
  4. Using assistance or a mirror, apply water over your donor area bandage. Be careful NOT to knock or touch your recipient area.
  5. Spend 5-10 minutes allowing the lukewarm water to soak into the bandage and over time this will begin to loosen.
  6. This will allow you to take off the bandage easily and dispose of it in your general waste bin.
  7. Once your donor area is exposed you can wash it. Use the palm flats of your hand (never your nails) to gently massage in baby shampoo into the donor area. It is normal for this to feel slightly tender on the first wash.
  8. Wash the shampoo foam off with lukewarm water.
  9. Air dry your donor area.
  10. You do not need to apply a new bandage to the back of your head.
  11. Continue to wash the donor area each day using the same technique until you are two weeks post procedure.

How to wash the recipient area:

On the 4th day following your hair transplant, you will be able to wash your recipient area. Here are the steps required for washing your recipient area:

  1. Ideally you want a shower head which you can hold in your hand and adjust the pressure, if not running the tap and using a clean cup will also work.
  2. Run the water ensuring that it is lukewarm, if you are using a shower ensure the pressure is set to medium/low.
  3. Using assistance or a mirror, apply water over your recipient area. Spend 3-4 minutes allowing the lukewarm water to soak into scabs around the transplanted hairs.
  4. Use the palm flats of your hand (never your nails) to gently massage in baby shampoo into the recipient area.
  5. Wash the shampoo foam off with lukewarm water.
  6. Always let the water do the work for you. Never try and actively remove scabs with your fingers as this may remove transplanted hairs with it. In time the scabs will come away with each wash.
  7. Continue to wash the donor area as instructed in the section above.
  8. Air dry your recipient and donor areas
  9. Continue to wash the recipient area each day using the same technique until you are two weeks post procedure.

A common question asked by clients new to anti wrinkle injections is, "What are anti wrinkle injections made from?"

Anti wrinkle injections are made from a toxin produced by a bacteria known as Clostridium botulinum. The toxin is called botulinum toxin aka botox. Clostridium botulinum comes in eight different strains A, B, C1, C2, D, E, F, and G—. It is only types A and B that are used in clinical preparations.

Botulinum toxin is used to treat many health conditions, including;

The bacteria Clostridium botulin was first discovered by a Belgian bacteriologist, Emile Pierre van Ermengem in 1895. This happened after he studied a fatal outbreak of botulism that was linked to a batch of tainted ham served at a funeral.

"Botulinum Toxin" was finally isolated in a crystalline form by Dr. Edward Schantz in 1946.

It wasn't until the 1970's that botulinum toxin was found to be useful to treat glabella lines. Scientists were experimenting on monkeys to treat strabismus (condition where the eyes are not properly aligned) with botulinum toxin. They noticed that it also reduced the wrinkles of the glabella ( also known as 11 lines) in monkeys.

Botulinum toxin works by preventing the release of acetylcholine which is a neurotransmitter responsible for muscle contraction. Without acetylcholine, the muscle contraction stops. Basically it stops signals from the nerves to the muscles. The result is a focal paralysis, the muscles relax and elongate and smooth the skin above. Botulinum toxin generally lasts in the range of 3 to 6 months depending on the individual.

By using the correct dosage the facial muscles can be partially paralysed, thus allowing for some facial movement, but not enough to demonstrate dynamic wrinkles. Most people wish to avoid having a completely frozen face where they cannot make any facial expression.

Apart from preventing the release of acetylcholine, botulinum toxin also prevents the release of pain and inflammatory mediators. This explains why botulinum toxin can be used successfully to treat migraine headaches.

A question we often gut asked at ACM is “ What is the best age to start getting Anti Wrinkle Injections?”

Believe it or not, people are now getting Anti Wrinkle Injections even when they do not have any wrinkles!  The reason people are starting to get Anti Wrinkle Injections at a younger age is as a preventative treatment to prevent the formation of wrinkles in the first place.  

It is often said that prevention is better than cure and this is certainly true when is comes to Anti Wrinkle Injections and wrinkles.  It is much easier to prevent wrinkle from occurring in the first instance than to treat them once they have set in and become static wrinkles.

Facial wrinkle are a little like bending a piece of paper. If you bend the paper enough times, eventually the bends will form a crease in the paper.  The same is true for facial skin. Dynamic wrinkles eventually turn into static wrinkles .

Botulinum toxin works by relaxing the muscles beneath the skin. This prevents the skin above from forming temporary dynamic wrinkles . 

This brings us back to the question, “what is the best age to start getting Anti Wrinkle Injections?” This will depend on a number of factors including including skin type and lifestyle.  If you smoke and spend a lot of time in the sun, you will be more prone to getting facial wrinkles than others.

By law the minimum age at which you can get Anti Wrinkle Injections is 18 years. It is possible to prevent dynamic wrinkle from progressing to static wrinkles by getting preventative anti wrinkle injections. At ACM we we suggest the best age to start would be in the late 20’s or early 30’s.  Most people come to us for botox once wrinkles have already set in, but it is now becoming increasingly popular for people to come to us to prevent wrinkles from forming in the first please.  The area of concern for most people is the forehead, as this is the most common area for dynamic expression lines to be seen.

If you are worried having a frozen and stiff appearance by having botox then fear not.  A doctor experience in cosmetic injectables will be able to provide the correct dose so that you can still have facial expression. The doctor will weaken the muscles slightly so that dynamic wrinkles are not able to form.

When deciding when you are ready to start getting preventative Botox, it is always a good idea to have a discussion with your doctor. By getting wrinkle injections at an early age you may over time develop a resistance to Botulinum toxin so that it becomes less affective and more Botox is required. Your immune system will develop so that the Botulinum toxin is metabolised more quickly over time.

“ What is the best age to start getting Anti Wrinkle Injections?” The age which you decide to get wrinkle injections to prevent wrinkles is certainly an individual decision. At ACM we suggest that if you wish to prevent wrinkle from forming you should first stop smoking and take good care of your skin by moisturising and wearing sunscreen when you go out into the sun. Limit sun exposure as much as possible. It is always a good idea to wear a hat in the sun. It is reassuring to know that if you are still worried about preventing wrinkles from forming, preventative botox is always an option.

As mentioned in our previous blog, it can take fourteen days for your new hair grafts to firmly implant into the scalp and become secure post your hair transplant surgery. For this reason, caring for your new hair grafts up until 14 days is crucial. The following is a simple guide, giving you "Tips for keeping your new hair grafts in good health";

Things you should do:

  • After the 5th day post surgery, you should wash your hair daily. This will help remove scabs and aid healing. Use only low pressure water and do not physically touch the grafts.
  • For the first 4 Days post surgery, you should spray your recipient area with saline solution regularly to help keep the grafts moist.
  • You may use Paracetamol after your surgery to help ease pain and also to help you sleep.
  • You should wear a loosely fitting hat for the first 14 days to protect the grafts from the sun.

Things not to do:

After 2 weeks have passed your grafts should be secure in your scalp. However, you should still treat the recipient area gently for up to one month.

A question we often get asked by our patients is, "How long after having hair transplant does it take for grafts to become secure?" This is a very serious question, especially considering the effort and expense you have gone to thus far.

For any hair transplant recipient it is important for them to be aware of how to take care of the new follicular grafts. The new follicular grafts are incredibly fragile immediately after having a hair transplant. The only thing holding the graft in place is a clotted plug of blood, which acts like a glue.

The hair transplant recipient must avoid any type of physical trauma to the recipient area. This means you may not physically touch the newly implanted grafts. For the first few weeks after your hair transplant you should sleep on your back, to avoid dislodging the grafts. Dehydration, lack of oxygen and high temperature may also affective survival rates of hair grafts. You should follow your surgeons after care instructions carefully to ensure your newly implanted grafts have the best chance of survival. You should avoid exercise after having a hair transplant as this may raise your blood pressure causing the recipe site to bleed and dislodge the graft. At day 5 post surgery the recipient area can be washed gently by low pressure water without touching the grafts physically.

Typically it takes 14 days for the newly implanted graft to heal and secure itself firmly in its new recipient location.

Lip filler injections are a cosmetic procedure performed by a qualified nurse or doctor to make you lips fuller and plumper. The topic of lip filler injections and keeping safe should be foremost on peoples minds when contemplating to have lip augmentation. Modern day dermal fillers are now the most common method of performing lip augmentation.  The most common type of dermal filler used today is made from a substance which is found naturally in your body, known as hyaluronic acid.  Because HA occurs naturally in the body there is much less chance of the body having an immune reaction to the filler product.

In the not too distant past, lip fillers were made from collagen, fat injections or implants.  These methods were often associated with greater chance of side effects. Hyaluronic acid fillers are considered to be much safer and have the capability of being dissolved if required.

Hyaluronic acid fillers have a consistency which allows the injector to have a greater degree of artistry. The cosmetic injector is able to control and manipulate the shape, structure and volume of the lips.  The results from lip fillers can typically last approximately six months before needing repeat treatment.

The benefits of Hyaluronic Acid Fillers include;

  1. Movement of the lips may cause lumps and bumps on the lips. These can easily be dissolved.
  2. Less bruising and swelling occurs with HA compared with other filler types.
  3. Injection of HA can be given gradually over a few sessions until the desired results can be seen.
  4. HA allows the doctor or nurse greater control over the volume of filler injected and lip volume created.
  5. Results can last up to six months but are not permanent. If you don’t like the look, it can be dissolved.

When and why did you open Advanced Cosmetic Medicine?

What do you think makes the clinic special?
I have 20 years medical experience as well as a fellowship with the Royal Australian College of General Practitioners, a Diploma in Aesthetic Medicine, a Diploma in Dermatoscopy and numerous other medical/aesthetic qualifications, but I believe my artistic eye and genuine love for my clients really sets the clinic apart.

DON’T OVER-DO IT

I advocate natural results – that is to enhance without overdoing it. I care about my patients’ wellbeing and I understand that over-treating the skin is not healthy anatomically or physiologically. 

WEAR SUNSCREEN

A common misconception is that you don’t need to wear sunscreen if it’s a cloudy day. You should wear sunscreen every day, even on a cloudy day or when you’re just lounging around. If you plan to sunbathe, lather your skin in sunscreen. I see teenagers lying on the beach without sunscreen to achieve that perfect tan. It’s dangerous to the skin and wrinkles could appear earlier. 

PREVENTION IS BETTER THAN A CURE

Getting regular skin treatments before you start seeing wrinkles is completely advisable and safe. Again, just don’t overdo it and make sure to go to a reputable cosmetic physician.

Cosmetic medicine is about enhancing your beauty and recreating the physiological aspect of your skin at a certain age. The great news is chemical peels have come a long way and are now so effective and with such little downtime, that you will be wishing you had discovered them sooner.  I had been struggling with a dull and uneven complexion and no matter how rigorous I was with my skin care; I just couldn’t seem to make a visible difference. So, when I was given the opportunity to try the treatment I jumped at the chance.

The chemical peel process

The treatment began by having my makeup removed and my face cleansed. Once this was done, Dr Targett began to apply the chemical peel to my face. While I laid there, Jayne the wonderful clinic nurse fanned my face to help relieve the effects of the peel. I would say the peel feels like pins and needles, coupled with a sunburn. It’s mildly uncomfortable but very tolerable and the application is over quickly. Dr Targett checked that my face frosted up, ( which shows that the process works ) frosting is exactly as it sounds- your skin takes on a frosted effect and she explained that the peel works by providing epidermal stimulation, and encourages collagen and elastin production to reveal visibly younger-looking skin. In a nutshell, the top layer comes off to reveal beautiful baby fresh skin below. Once the treatment was finished Dr Targett applied the restoring ointment to my face as well as some sunscreen – and I was ready to go.

Post Treatment

The peel stays on for 24 hours post-treatment and then you can wash it off with warm water. It’s super important to wear sunscreen daily and to reapply during the day as the skin is more sensitive to the sun. Additionally, you don’t resume your normal skincare routine until all your skin has peeled completely and instead apply the restoring ointment throughout the day – especially when the skin feels tight – which it does by day two. My skin started to peel on day two and by day three it really kicked off – with baby fresh skin revealed below. My skin then kept peeling solidly for five days and stayed a pinkish colour for about 7 days post-treatment. During this time, I continued to apply the restoring balm and sunscreen religiously.

The verdict

After my skin had finished peeling it was absolutely GLOWING. My pigmentation and freckles also seemed to have diminished significantly and the overall colouring was much more even across my face. My pores seemed less noticeable and my creams and serums seemed to absorb more quickly.  I can see a huge benefit in getting this treatment done a couple of times a year.

I can now confidently say I am a Chemical Peels convert – my skin has never been better and the change in tone and texture was immediate and obvious.

Many of us have had at least a dalliance with acne. We are told that getting acne is a rite of passage, part and parcel of puberty, at the end of which is adulthood. But the problem of acne is twofold – skin blemishes that fade eventually and permanent scarring / sunken scars.

Severe acne can sometimes create deeper-than-normal scarring, resulting in visible depressions in the skin surface that cannot be removed by creams, lotions or scrubs. This is because of abnormal healing of the skin after acne, the haphazard arrangement of scar tissue bands underneath tethering the skin to the underlying tissue to form sunken scars. This type of scarring can only be treated with subcision, which is one of the best procedures for scar correction. Subcision entails placing a special needle underneath the scar and breaking down the tethered tissue horizontally. This breaks the scar tissue bands beneath the skin. To make the scar flush with the surrounding skin, the skin level of that scar will have to be raised by replacing the lost volume, which can be achieved in several ways. First, collagen production can be stimulated with Laser Genesis or Factor 4 treatments. If you want an instant improvement in the appearance of your scar, minute amounts of dermal fillers can be placed under each scar after subcision to create a smooth skin surface. Another option is biostimulation fillers; unlike traditional fillers, biostimulation dermal fillers not only deliver the desired immediate results but also induce the body to produce its own collagen, so even after the dermal filler substance has been absorbed by the body, the space under the scar will be filled with natural collagen.

Subcision has a demonstrable effect on rolling scars and boxcar scars, but not ice pick scars because of the structure of the scar tissue. Furthermore, depending on the severity of the scarring and the amount of collagen production by the body, it may take multiple treatment sessions to address all the scarring sites and revisit partially elevated scars. This treatment is conducted with a sharp needle and numbing lotion or local anaesthetic may be required to minimise discomfort. Due to the nature of the procedure, there may be some swelling and bruising post-procedure, and two to three days of downtime should be allowed. You should also avoid sun exposure for seven days after the treatment to reduce sun damage while your skin is healing.

Acne is a skin condition most associated with teenagers and puberty, yet these days, this problem is plaguing an ever-increasing number of preteens and people who have reached adulthood.

The most common treatments for acne include topical creams and gels, antibiotics, birth control pills and, in more severe cases, isotretinoin (which used to be known as Accutane). Dermatologists and doctors are likely to recommend these options as they are known to be effective, but these treatment methods only suppress the symptoms of acne without treating the root cause. Long-term use of antibiotics has been proven to result in lower gut bacteria diversity, which impacts our immune system and metabolism. Oral contraceptives are limited to women who have entered puberty and only address one acne-causing factor – excess sebum, the naturally-producing oil that moisturises our skin. Perhaps worst of all are the side effects of isotretinoin, which include and are not limited to dry lips and mouth, sun sensitivity and complications during pregnancy resulting in birth defects.

With the slew of problems brought by conventional acne medications, we have turned towards more natural alternatives for a better solution. There is now growing scientific evidence to suggest what we’ve known or heavily suspected for a long time – what we eat can indeed affect our skin. So, what changes can we make to our lifestyle and eating habits to improve our complexion and reduce acne?

The Paleo (short for Paleolithic) diet, also known as the Stone Age or caveman diet, appears to be quite effective at reducing acne. The idea behind this diet is that we should try to understand and recognise how different food affects our bodily functions and adapt accordingly. For instance, research has shown that increased consumption of milk and foods with a high glycemic index (GI) or glycemic load (GL) – usually highly processed food such as refined flour and excessively sugary food – correlates with acne formation. That means by avoiding or simply cutting down on our intake of grains, legumes, refined sugars and dairy, we should experience reduced sebum production, improved insulin sensitivity and less follicular inflammation – basically speaking, we should be getting less acne. There is also evidence that, for some, avoiding gluten can have a positive effect on their skin.

It is important to note that adopting a Paleo diet doesn’t mean having to go cold turkey on all processed food. It simply means incorporating as much whole, unprocessed foods like vegetables, fruits, berries, free range meats and wild caught fish into your meals as possible. It should come as no surprise that minimally processed food is better for us overall. After all, most of us can recall that sluggish feeling after eating fast food. You might find that by switching to a low-carb, low-sugar, gluten-free and lactose-free diet, your skin feels less greasy over time or that you tend to break out less.

One last thing to remember is that when it comes to diet, trial and error is key. What works for one person may not work for another, but with time and some experimentation, you will find the type of diet that works best for your skin.

In the Fall of 2016, my jaw was half-locked and dislocated for some unknown reason. To make the story short, the physician referred me to a TMJ physiotherapist and my dentist recommended a night guard. The treatment was working but sadly, the TMJ disorder comes back every now and then since its first occurrence. Whenever I feel soreness on my jaw, I know it’s back and I have to go to the physiotherapist again to fix the dislocated jaw and to ease the pain. I came to the point where I would be in so much throbbing pain that I realized I needed to take care of the pain.

According to Neuroscience Research Australia led by Dr. Sylvia Gustin, “The challenge of developing such [pain] treatments has led to more research on the brain’s role in chronic pain.”

In an interview with ABC in 2016, Gustin’s research team said, “At the moment we have focused our work to two areas in the brain, one is called the thalamus—the other is the prefrontal cortex. Described as the ‘border in the brain’, the thalamus acts as the gateway between the spinal cord and higher brain centres.”

“When you sustain an acute injury, there is an opening in the thalamus for information to pass through from the affected body part to the brain. This is very important because then we need to heal, we need to relax, we need to look after ourselves. After an acute injury is healed, we know that this border should actually close.”

When researching people who experience chronic pain, Gustin identified a key neurological difference: the opening in the thalamus remains open long after acute pain is gone. They also found a decrease in the volume of the thalamus, resulting in a decrease of a specific neurotransmitter: gamma-aminobutyric acid, or GABA.

So, what does this mean? Gustin explained it means that people with ongoing pain, this border is always open. Every signal gets amplified and it results in the experience of pain.

Moreover, researchers also found people with chronic pain experienced a reduction in the volume of their prefrontal cortex—the region of the brain that is understood to regulate emotions, personality expression, and social behaviour.

This results in a further decline in the neurotransmitter GABA.

Gamma-aminobutyric acid (GABA) is the brain’s most abundant inhibitory, or ‘calming,’ neurotransmitter. While GABA is known primarily for its ability to put us in a relaxed state, it actually plays a crucial role in regulating many aspects of mood, attention, cognition, and sleep. GABA deficiency symptoms may involve any of these functions and include depression, anxiety, insomnia, and more.

The above explanations are the reason why I started to check other alternatives for the throbbing pain in conjunction with the yearly physiotherapy, plus, I didn’t really like using the night guard.

Last year, I discovered anti-wrinkle injections through Dr. Targett for TMJ pain relief. While anti-wrinkle injections are known for easing the look of wrinkles, it also helps to relax the muscles, and in my case, I was hoping to soothe my TMJ disorder – and it worked!

The anti-wrinkle injection works to relieve jaw tension and pain. It can eliminate headaches, teeth grinding, and lockjaw by relaxing the muscle that unconsciously creates such stress. This will also keep the jaw muscles from looking defined and the jaw from seeming wider, which is an additional advantage.

How Does It Work

The entire process only takes a few minutes, with a few injections on either side of the face, directly into the jaw muscles. Dr. Targett told me the result would be noticeable after a week or two. To my surprise, the result was only after a few days (not even a week), the throbbing pain stopped that I completely forgot about it.

The injections go into the masseter muscle. The masseter muscle is one of the muscles activated during chewing. It’s located at the angle of the jaw. The treatment takes roughly five minutes.

We all know that anti-wrinkle injections are used to relax the facial muscles that cause fine lines, the maximum effect is seen in seven to 10 days. Similarly, when they are used to relieve the pain of TMJ and teeth-grinding, you should expect some pain relief in around a week. When the injection is used to reduce masseter muscle hypertrophy cosmetically, the maximum effect is not seen for between six weeks and three months.

Anti-wrinkle injections can be used both functionally to weaken the masseter muscle, which is the main teeth-grinding muscle, and improve the pain of TMJ from teeth grinding, or cosmetically to thin the masseter muscle and take away a square look to the angle of the jaw from a hypertrophied or overgrown masseter muscle, softening the look of the lower face.

There’s no downtime, no residual pain, and I went back to my regular activities without any marks or bruising.

Within the next few days, I started to notice far less clenching, especially when I’m driving or when writing my articles. I didn’t have any issues even when I went to my dental hygiene appointment which has always been a challenge since the TMJ disorder occurred. It’s been eight months since the last treatment and it’s just lately that I’ve been noticing the clenching again, which is normal because the injectable treatment lasts for at least six months.

There are questions you can ask during your consultation with a qualified medical doctor which will help you make an educated decision on whether or not to go ahead.

Question 1: Who will be performing my procedure?

Make sure the person performing the procedure is a qualified and registered medical doctor or a nurse who is acting under a doctor’s supervision. The national register maintained by the Australian Health Practitioner Regulation Agency lists all persons who hold registration with the Medical and Nursing Boards of Australia and can be freely searched on the AHPRA website.

Question 2: How much experience do you have in this type of procedure?

Doctors must have extensive knowledge of the facial anatomy, as well as the required training and experience.

Injecting filler into the wrong area of the face may have serious consequences, including blindness or even death.

Question 3: What product will you be using? Is it approved by the TGA?

Due to legislation and regulations governing the advertising of therapeutic goods, product information such as the brand name and substances cannot be advertised and so this information is only available if you ask.

Kylie Jenner at just 21 years of age has made a huge impact on the cosmetic injectables industry and has helped make the lip flip ever popular. The way her image has dramatically changed her look, her life and her popularity sends out a huge message to the masses and millions have followed her.

With so many different lip treatments on the market, cosmetic dermatologists and plastic surgeons are having to craft unique lip treatments to meet their patients big lip desires and needs. A few units of anti wrinkle injection are placed strategically around the lips which adjusts the way that the muscles relax. This means that your lips curl out a little to enhance the cupids bow – and give your lips a great sexy look.

Anti Wrinkle Injections  or Fillers?

The look can be done with anti wrinkle injections or can be combined with fillers. It gives lips an understated pout by flipping the line of the lip, and gently rolling the lip outward. If you use too much anti wrinkle injections, the problem is that your mouth simply will not move. It is hard to eat and drink. And the longer you want the treatment to work the more anti wrinkle injections you need to use. So, it will look great – but forget about eating or drinking.

To maintain the look of a lip flip, you need to have regular treatments. The lip flip is a great way to see if you like the way your lips look before you commit to using filler. If you would like a more subtle look you can start with a lip flip and then add some filler when anti- wrinkle products begin to wear off. The look you go for is entirely up to you – you can choose full volume or something much more subtle.
