Terms & Conditions Agreement

By signing this agreement the client agrees to abide by the below terms and conditions. The doctor agrees to treat you based upon your agreement to these terms and conditions. The terms and conditions agreement shall remain in place from the date below

Informed Consent Policy

Cosmetic procedures can only be performed after obtaining proper informed consent from the patient (you). ACM is not able to provide you with an exhaustive list of side effects, risks and, patient outcomes. If you are concerned we recommend that you seek a second opinion or conduct further research before consenting to a procedure with Advanced Cosmetic Medicine.

Post Procedure Care Policy

Advanced Cosmetic Medicine/Dr Targett will provide you with post-procedure/aftercare instructions after your procedure. It is a requirement that you follow these. As part of this agreement, you are required to follow these instructions. Should you experience any reaction to a treatment that you believe to be out of the ordinary, please contact Advanced Cosmetic Medicine immediately.

Half mls of Dermal Fillers – Policy

It should be noted that at Advanced Cosmetic Medicine, we do not offer half mls of dermal fillers. Should you require less than one ml of dermal filler, you are required to pay for the price of one ml. The remainder of the dermal filler will be discarded or your doctor may suggest other areas for the product to be placed for enhancement. You are under no obligation to use the remainder of the product.

48 Hour Notification of Cancellation Policy

At ACM we require clients to notify us 48 hours before their appointment should they be unable to attend. This allows other clients to avail this appointment space.  If you fail to attend your appointment or provide the required cancellation time, ACM reserves the right to apply a fee of  $80 to your account. Two days before your appointment you will receive an sms message asking you to confirm your appointment.  It is important that you respond to this text message.

Payment at Time of Service – Policy

You are responsible for ensuring that payment is made for your account by either cash, Eftpos or credit card. If for any reason your chosen method of payment is declined, we may enter into agreed bank transfer as agreed by informed written consent at the time of your appointment.

Procedure Fees Policy

Procedure fees vary according to the individual characteristics of the patient, the practitioner performing the procedure, the products involved, technology involved, etc. It is the client’s responsibility to be aware of the cost. Prices are on the clinic’s website and the cost of the procedure will be confirmed at your initial consultation.

Refund Policy / Realistic Expectations Policy

It is our wish for every patient at Advanced Cosmetic Medicine to leave with a feeling that they have had a positive experience. For this to occur patients must have realistic expectations of their treatment results. Whilst it is good to be positive and optimistic about your expected results, patients must realise that Cosmetic Medicine is not an exact science, rather it is both a science and an art. Every human body is different and may react differently to cosmetic products. Due to a large number of variables involved is not possible to provide a guarantee that you will achieve the treatment result that you desire. Advanced Cosmetic Medicine will work in collaboration with you to make every reasonable effort to achieve the aesthetic result you are after. However, there are no guarantees. Often several treatments may be required before your desired result are achieved. You (the patient) will be required to be responsible for all costs of all treatments required to achieve your desired results. Refunds will be provided where required under the Australian Consumer Law. By signing this service agreement the client agrees to the following; The doctors’ consultation fee is non-refundable. The cost of products used in your treatment is non-refundable.

Review of Procedure Policy

Advanced Cosmetic Medicine will inform you at your appointment when/if a follow-up appointment will be required. It is important to attend all appointments post-procedure/s so that we are able to assess your results. It is your responsibility to inform the clinic if there are any signs of serious complications.

Photography Policy

Every patient will be required to have before and after treatment photos taken. This is an important record to determine how successful the treatment has been.

Suitability for Treatment Policy

Advanced Cosmetic Medicine does not accept all patients for treatment. Some patients are unsuitable for the procedures performed at Advanced Cosmetic Medicine. Advanced Cosmetic Medicine reserves the right to decline or cancel further management of any patient if in the clinical opinion and/or the medical practitioner if you are unsuited for further care.

Side Effects and Complications Policy

All cosmetic procedures carry associated side effects and complications which you will be made aware during your consent process. While we will inform you of the most common side effects of your procedure/s, this list may not be exhaustive. Should you be concerned about any side effects, we suggest you ask Dr Targett questions during your consultation and conduct further research on your own. By accepting the risk of a procedure/treatment and the side effects/complications that may occur you are also accepting liability for the cost of treating these risks/complications. Side effects and complications of a cosmetic procedure may have long term health implications that may affect your ability to earn an income. Side effects/complications of cosmetic procedures may also affect the way you look and may impact on your confidence and ability to be seen in public. Patients must accept these risks when they undertake to have a procedure performed.

Post Treatment Care Instructions Policy

You will be emailed post-treatment care instructions following your treatment. It is most important that you follow these instructions for you to achieve optimal results. Should experience symptoms after your treatent that you believe to be out of the ordinary, please contact ACM as soon as possible.

Compliants / Online Reviews Policy

To be accepted as a client at ACM you must agree to refrain from posting online reviews that are defamatory to Advanced Cosmetic Medicine. If you are unhappy with any experience/treatment that you have received at Advanced Cosmetic Medicine, we ask us to please contact us immediately. We will address all complaints in an amicable manner. Before making a complaint, please consider if you have complied with the “Realistic Expectations Policy” as above.  We also ask you to put all complaints in writing and send it to us by email. We value all our clients and want all our clients to have a positive experience at Advanced Cosmetic Medicine.We value your complaint as a means of improving our service to our customers. AHPRA does not allow any online reviews that reflect upon the clinical aspects of any treatment or competence of the treating doctor.
